When we migrated to the new updated webpage, we also migrated all old accounts over.  The problem is that some data, specifically, your address, title and degree were not able to make the move.  If you have not updated your address or other information in the "My Account" section, then the documents will display blank information.  Once you update this, the documents should display correctly.
